Jean Davis was a dedicated member and president of the Fort Worth AAOHN as well as the Vice-President of the Texas AAOHN during the 1980s. She also served as the OHN for the Miller’s Insurance Company for many years. The Jean Davis Scholarship Awards are in honor of the hard work and dedication she showed to Occupational Health Nursing.
Purpose
- To strengthen nursing practice by providing financial assistance to an individual enrolled in an academic program that will lead to either an Associate’s, Bachelor’s, Master’s, or a Doctorate’s degree.
Amount of scholarship:
- Undergraduate: $1,500
- Graduate: $3,000
Eligibility
- The applicant must be enrolled in/or accepted into a degree program. This will include an LPN /LVN or RN seeking a higher degree that will lead to Associate’s, Bachelor’s, Master’s or a Doctorate degree in Nursing.
- The applicant must show being an active contributing TSAOHN member.
- The applicant must have a current LPN / LVN or RN License to practice.
- The applicant must be a citizen of the United States or a permanent resident of the United States.
- The applicant may be either a full time or part time student in nursing (minimum of 3 hours a semester)
Instructions
- Complete attached nomination form and submit to the Director of Awards/Recognition/Benevolence via the online application portal. Deadline for Nominations has been extended thru Nov 1, 2023. (Nomination forms received after the due date specified on the form will be disqualified.)
- Two (2) Letters of Recommendation that address the nominee’s achievements, professionalism, and contributions to the profession. (Letters of Recommendation received after the due date specified on the form will be disqualified.) Please send those as attachments to admin@tsaohn.org
- The applicant must submit proof of enrollment in or acceptance to a program leading to a degree.
Selection Process
- The selection of the nurse shall be made by a judging committee selected by the Director of Awards/Recognition/Benevolence. The committee consists of three (3) occupational health nurses. Members of the judging committee may serve for two (2) year terms. Committee members may be eligible for re-appointment.
- Identifying information will be omitted from copies of nominations prior to sending them to the judging committee.
- Prior to the presentation of the scholarship, the recipient selected to receive the award shall not be revealed except to the TSAOHN President or others as may be deemed necessary by the Director of Awards/Recognition/Benevolence.
Judging Criteria
- 50% Career Goals
- 30% Outstanding Contributions to the profession of occupational health.
- 20 % ‘Reference Letters’ addressing leadership initiative, motivation, professionalism, creativity, commitment and contribution to the profession.
