Directions: All information on this form must be completed, either on-line or a hard copy. If completing a hard copy, please return the form to your chapter president, your Election Advisor and the TSAOHN Administrator. The forms will then be reviewed on the TSAOHN website. This acceptance Form A must be received no later than December 9, 2021. Nominees with incomplete information or late entries may not be accepted. Please send a digital photograph suitable for use on the TSAOHN website and to send out to the HTAOHN membership. On December 10, 2021, all nominee photographs and bios will be highlighted on the website and via member email and will remain there until after the voting process has ended. Voting is December 10-16, 2021.
