Position Description:
Summary:

Provides medical case management services for
employees' occupational and personal injuries/illnesses as well as
Americans with Disabilities Act (ADA), Family Medical Leave Act (FMLA)
and Long Term Disability related cases.

Manages the Return to Work Policy and the Drug and
Alcohol Policy processes as well as any related issues.
Essential Duties/Responsibilities:
- Ensures accuracy and completeness of OSHA and
other required record keeping.
- Ensures company records, forms and processes are
in compliance with HIPAA's administration provisions that address
the security and privacy of personal health data.
- Consults with physicians, medical service
providers, employers and employee and family members to coordinate
medical services.
- Manages return to work process for injured and/or
ill employees in a cost effective and timely manner.
- Provides timely information to management about
employees' health restriction status and anticipated return to work.
- Coordinates with Human Resources and management
should an employee fail to be compliant and/or a medical provider
fail to provide timely and or complete information.
- .Maintains on-going evaluation of treatment,
medical services and return to work plans within established
protocols and disability duration guidelines.
- Works closely with the ill and/or injured
employee and medical providers to achieve timely treatment planning,
implementation and follow through.
- Establishes a professional rapport with the
employee, family members, management and medical providers to
produce creative and cost effective solutions to achieve maximum
medical recovery and efficient return to work.
- Answers the "HURT line" emergency medical phone
calls based upon rotating on-call schedule.

Coordinator
Position Description:
The Occupational Health Nurse is responsible for the
coordination of the employee health program for the St. Luke's Campus. This
includes, but is not limited to, job placement evaluations; assessment of
employee injuries or illness including blood and body exposures; and the
provision of employee immunization and screening programs. The Occupational
Health Nurse will respond to both internal and external contacts in a
courteous, professional manner using good judgment and discretion to provide
assistance or to arrange for contact from the appropriate party.
Primary Accountabilities/Responsibilities:

Reviews health history and evaluates employees or
employee candidates who are at risk for injury due to work-related
duties.

Refers employees/candidates who are at risk to the
Employee Health Physician Consultant, to determine individual's ability
to perform prospective job duties as needed.

Coordinates and maintains employee immunization and
screening programs.
-
Evaluates any employee presenting to Occupational
Health Services for injury or illness, providing treatment and/or
referral to a physician, documenting actions and recommendations;
coordinates appropriate care after hours for employee injuries or
illness.

Reports unsafe work practices to the Safety Officer.
-
Evaluates employee blood and body fluid exposures,
according to policy, obtaining test results, following up on abnormal
values, and ensures appropriate counseling and treatment is provided to
the employee.
-
Assists in the management and coordination of the
modified duty program, by identifying opportunities for modified duty,
and monitoring lost-time injuries.
-
Ensures that TWCC1 and TWCC6 forms are processed
according to timelines established by the Texas Workers' Compensation
and Claims Management Company.
-
Ensures that TWCC wage statements on all injuries
that result in greater than seven days of lost time from work are
processed in a timely manner.
-
Assists employees with questions concerning Worker's
Compensation Benefits.
-
Appropriately categorizes and logs all OSHA
reportable injuries on the OSHA form 300, according to OSHA guidelines.
Maintains employee confidentiality in accordance with Health System
policy.
-
Performs other duties as assigned or requested by
supervisor.
-
Follows all safety rules while on the job. Reports
all accidents promptly and corrects minor safety hazards.
Position Requirements:
MINIMUM REQUIREMENTS:
-
LVN or RN Minimum 5 year experience in applicable
setting.

Recent experience must be acute care environment.
-
Certification in Occupational Health preferred and
the ability to become certified.
